Formulary Changes
Notice of changes to the formulary made during the coverage year will be posted at least 60 days before the date the formulary change becomes effective. Changes may include:
- Drugs removed from the formulary.
- Addition of prior authorization, quantity limits and/or step therapy restrictions on a drug.
- Drugs moved to a higher cost-sharing tier.
If the Food and Drug Administration deems a drug on our formulary to be unsafe, or if the drug's manufacturer removes the drug from the market, we will immediately remove the drug from the formulary and posting of notice may be retrospective.
